Download the notary form here. (do not sign it until you are in front of the notary!)
Prizes are ranked in order of the most hoops donated AND the most money raised! Both of these will be considered for all contest applicants.
But here's some guidelines:
- You must donate quality hula hoops. Then download the soon to be released form stating how many hoops you gave away on World Hoop Day / Weekend (August 8, 2008 - August 11, 2008). Get this form notarized and return it to the WHD HQ.
- Submit the funds you raise to us via Google Checkout, PayPal, or Check.
- These two numbers will be what the contest is based on.
Deadline for all monetary donations and notary forms is August 31, 2008. Numbers will be tallied and awards will be distributed in September 2008 with winners announced in our newsletter!
